This Privacy Policy (“Privacy Policy”) outlines how GuidingPatients, Inc. and our affiliated companies (“GuidingPatients”, “we,” “our,” or “us”), use Personal Information (defined below) that we process in connection with you use of our website and all software, services, extensions, subdomains, and application programming interfaces (collectively, our “Service”).   • Employment information, including title and relation to Facility (if applicable);
  • Characteristics of protected classifications under state or federal law, such as age, sex/gender, marital status, medical condition and disability information;
  • Information you provide on our Service for yourself or another person (i.e. a Potential Resident), for example your location, zip/postal code, mailing address, fax number, and email address;
  • Financial information, including the Potential Resident's budget for post-acute care facility options and whether the Potential Resident receives Medicare or Medicaid;
  • Feedback you provide to us about our Service;
  • For Potential Residents, their gender; household income; budget; care needs, including general health information and medical conditions; as well as other information you may choose to voluntarily provide, such as marital status, religion, or ethnicity; and
  • Communications you send to us via any method or channel.

3. When Personal Information is Collected

In connection with our provisions of the Service to you, we may request Personal Information from you. The exact Personal Information that we collect from you may vary depending on the purpose.
